Mission/Vision:
The Saddle Lake Coordinated Home Care Program is a comprehensive integrated and coordinated home care and assisted living program to assist Saddle Lake First Nation peoples to achieve and maintain health, well-being and personal independence in the community utilizing a holistic approach.

Goal:

Assist individuals to live independently by providing required health and support services and by strengthening the support provided by the family and community.

Description of Services:

1) Essential Services

a) Home Care Nursing services and;
The basic level of service that is provided to the community of Saddle Lake includes:

Detailed client assessments, completed by Health Professionals:

  • To gather information on the clients need for service, their support networks, and the appropriateness of services available

Case management process that provide:

  • Ongoing evaluation of appropriateness, effectiveness, and eligibility for care

Professional Nursing services, encompassing:

  • Direct personal care to eligible clients
  • Operational process for Records management, data collection, quality programming
  • Access to required medical equipment, medical supplies through On-Site Occupational Therapist

Nursing Services:

  • Home Visits
  • Assessments (initial, care plan)
  • Case reviews/client conferences
  • Monitoring Medications
  • Ordering Medical Supplies
  • Wound care
  • Receives/screens referrals from institutions, community agencies, and individuals
  • Teach personal care to individuals families & caregiver(s)
  • Refers clients to appropriate resources
  • Foot care/Diabetic Foot Care
  • Palliative Care
  • Suture Removal
  • Lifeline Removal
  • Hospital Discharge

2) Supportive Services (Assisted Living):
Once the essential service elements are met, the Home Care program may decide if any of the following supportive service elements may be given:

  • Household management
  • Home making
  • Laundry
  • Transportation
  • Minor Home adaptations for client safety
  • Meals on Wheels

Eligibility Requirements:

Before you can receive Home Care nursing services, you should:
Be a resident of the Saddle Lake Cree Nation
Be under the care of a doctor
Discharged from Hospital early and require care, such as:

  • Wound care
  • Medication
  • Order Medical Equipment

Assisted Living: 

  • Clients are assessed before supportive services are initiated
  • Active Home Care clients only without family support will be assisted until stable
  • Aids to Daily Living is our PRIORITY

 Admission:

Referrals to the Home Care Program accepted from hospitals, doctors, family, friends, or concerned citizens.

 Initial Assessment:

Once a potential client is referred to the Home Care Program, the Home Care Nurse HCN does a preliminary assessment of the person to see if they qualify for assistance. Client eligibility varies with the initial assessment done by the HCN.

Types of clients’ eligible to receive continuing care services in the community:

  • Seniors, 65+
  • Disabled clients
  • Chronic
  • Discharged from hospital early and requires care
  • Wound care
  • Medication
  • Order medical equipment

Consideration for Admissions:

  • Lives alone or with an equally disabled spouse
  • No informal support system
  • Functional limitations
  • Frequent hospitalizations (3-6 months)
  • Requires service to facilitate discharge from the hospital
  • Conditions which indicate further deuteriation expected in the near future
  • Memory impairment
  • Acute & palliative care
